1. Goal Identification

2. Materials Request

After developing an activity sequence, contact WSU Extension Educator to request any required elements or props.

3. Preparing your Team for the Day

Full Value Contract

Complete this form to prepare your challenge team. The consent/release must be filled out and signed by all participants (or their parents, if they are minors) BEFORE they are allowed to participate in the WSU Challenge.

- What to Bring
    • First Aid Kit
    • Sturdy Shoes (no open-toe shoes)
    • Bug Repellent, Sun Screen
    • Layered Clothing consistant
       with current weather conditions
    • Water (food, if it's an all-day event)

 

Participant Insurance

If you are a 4-H sponsored group, you are also responsible for acquiring 4-H special activity insurance coverage from American Income Life. Click on "Apply for Coverage Online."

4. Day of the Event

You must have a working cell phone that can receive a signal while facilitating the event.

Make sure you have a copy of everyone’s consent / release / risk of exercise forms with you at the event.

Accidents must be reported to the appropriate entities within 24 hours.
As soon as possible, make sure to fill out the an incident report form and return to the Extension Educator.

5. Following the Event

• Complete the Challenge Use Report Form - Link

• Store your Consent Forms for 7 years

• Mail in any Incident/Accident Reports to address below.
